Delivering individualised NDIS services across Greater Geraldton, WA

Empowering Services, Tailored To You

Experience the transformative power of personalised support from a team that cares.

We support self-managed and plan-managed participants.

1:1 Supports

Our one-on-one supports are about more than just assistance; they're about fostering a relationship of mutual trust and respect. Tailored to the specific needs of each person we serve, these supports may include personal care, assistance with daily tasks, or help with mobility and travel. The goal? To empower you to achieve your fullest potential and maintain your independence.

Community Engagement

Engagement within the community is a cornerstone of a full and meaningful life. At Ubuntu Service, we help people living with disability to connect with their community, fostering a sense of belonging and participation. This might involve assistance with attending local events, participating in group activities, or simply enjoying a day out in beautiful Sydney. The result? A richer, more connected life experience.

Building Life Skills

The ability to carry out daily tasks independently can significantly enhance one's sense of self-confidence and accomplishment. Our life skills development services are aimed at supporting people in learning or improving skills that are essential for daily living, enhanced relationships and community engagement. This could include money management, cooking, employment skills or using public transportation. Our aim is to boost independence and promote a sense of empowerment.

Community Nursing

Our community nursing is provided by experienced, qualified nurses who understand the unique needs of those living with disability. We offer a range of services including complex care management, medication administration, PEG feeding and wound care, all provided in the comfort and familiarity of your own home. These services not only support health and well-being, but also provide peace of mind to you and your loved ones, knowing that professional, compassionate care is readily available.

  • How do I apply for the NDIS?

If you meet the eligibility criteria, you can apply for the NDIS by completing an Access Request Form. More details can be found on the NDIS application page.

  • What are the eligibility requirements for the NDIS?

To be eligible for the NDIS, there are three main criteria a person must meet:

    1. Age: You must be under 65 years of age when you first apply for the NDIS.
    2. Residency: You must live in Australia and be an Australian citizen, hold a Permanent Visa, or a Protected Special Category Visa.
    3. Disability requirements: You must have a permanent disability that significantly affects your ability to take part in everyday activities, or a developmental delay.

You can find out more about the eligibility criteria here.

  • How does the NDIS planning process work?

The NDIS planning process is a collaborative effort between the participant and the NDIA. Once you are deemed eligible, a plan will be created which outlines your supports and services that will be funded through the NDIS. This plan is tailored to your individual needs and goals, and includes three types of budgets: Core, Capacity Building, and Capital. You can learn more about the planning process here.

  • What are Service Agreements and why are they important?

Service agreements are contracts between an NDIS participant and a service provider, like Ubuntu Service. They outline the type of supports provided, the cost of those supports, and how and when supports will be delivered. Service agreements are important as they ensure both parties understand their rights and responsibilities. You can manage your service agreements through the myplace portal.

  • What makes Ubuntu Service different from other providers?

One of the key differences is Ubuntu Service's commitment to consistency. Clients will always have the same support team, which fosters a sense of familiarity and trust. Additionally, Ubuntu Service stands out because of our team's clinical experience, providing a level of expertise and understanding that greatly benefits our clients and their families.

  • Will I always have the same support team?

We understand the power of consistency and the importance of building strong relationships between participants and their support teams. Therefore, they aim to ensure participants continue to work with the same support team throughout all the stages of your life.

  • How do I get started with Ubuntu Service?

Getting started with Ubuntu Service is a supportive and personalised process. Even if you don't have NDIS funding in place, we're here to help. We can assist you in understanding your eligibility for the NDIS, guide you through the application process, and help with the creation of your NDIS plan.
